About Our Practice

Health Service Alliance operates several Community Health Centers in San Bernardino County.

Health Services Alliance provides comprehensive and integrated primary medical, dental, mental health, and substance abuse treatment services to low-income residents in southwest San Bernardino County, California. The target population of HSA includes hundreds of thousands of residents living below 200% of the Federal Poverty Level (FPL) in the cities of Chino, Montclair, Ontario, Pomona, Upland, Palm Desert to the east, and Rancho Cucamonga to the north.

Meet Our Leadership

Health Service Alliance’s leadership is composed of the most experienced, innovative, and detail oriented professionals.

James M. Lally, DO, MMM

Chief Executive Officer

Dr. Lally brings to the project the experience and knowledge of years of high quality medical care and service to the community.   He recently departed Chino Valley Medical Center where he held the position of Chief Medical Officer since 2004 and worked closely with administration and medical staff to insure a very high performing facility. 

Board certified in Family Practice, with certificates of added qualification in Sports Medicine and Geriatrics, he was responsible for the day to day operations of Chino Valley Medical Center and Montclair Hospital Medical Center.  Dr. Lally also served as Director of Medical Education for internship, family practice residency and sports medicine fellowships and served on the Board of Trustees for both hospitals.  He is recognized in the local community as a charitable and giving physician, who leads by example.  Early to work and late to leave, he serves as a role model to his management and staff as someone willing to “walk the walk”.  He serves as team physician for the USA Shooting Team and is its Immediate Past President of the Board and is Chairman of the Medical Committee of the International Shooting Sports Federation as well as a member of the International Olympic Medical Commission. 

Locally, he oversees the sports medicine programs of the four local high schools in the Chino Valley Unified School District.  He is a member of the YMCA Board of Directors and is a past board member of Chino Youth Boxing. In 2016, he was recognized as the Physician of the Year for the American Osteopathic Foundation having been selected from over 125,000 eligible physicians and was selected for the Corporate Division, Inland Empire Spirit of the Entrepreneur award.  In 2017, Chino Valley Medical Center was recognized as a Truven Top-100 Hospital for the sixth year in a row.

Jeffrey Cox, MBA

CFO/CIO

Mr. Cox joined HSA in 2018 as CIO and is responsible for day-to-day management of the corporation’s information systems software and hardware functions.  Mr. Cox previously served as Regional IT Director for Chino Valley Medical Center, where he was responsible for leading a team of 18 analysts and software engineers for four facilities. His areas of responsibility included strategic project planning, resource management, customer service, and technology leadership and support.  He established short and long-term goals, including Meaningful Use attestation, IT cost savings initiatives, and continuous process improvement in hardware and software available to clinicians and medical staff in an effort to provide comprehensive and cost effective care to patients. 

Over a twelve-year career at Chino Valley Medical Center, he served as PC/Network Operator, PC/Network Specialist, Regional Systems Administrator, and Regional IT Manager before becoming the Regional IT Director.  He is skilled in managing the full spectrum of project management responsibilities and has extensive expertise in each stage of the hardware, software, and networking implementation lifecycle.  Mr. Cox holds a BS in Business Administration from Westwood College and an MBA in Management from National American University.  Additionally, he is a Certified Security Compliance Specialist and a Certified HIPAA Professional. 

Christen Grelling, MSN, FNP, PHN

Chief Clinical Officer

Christen Grelling most recently served as Administrator/Chief Nursing Officer/Risk Manager for Chino Valley Medical Center, where she provided strategic leadership for functions and services, along with direct oversight of day to day operations implementing improvements as appropriate.   She ensured that the hospital had a high level of competency within the clinical staff through skills validation, didactic training, testing, and one on one education. She chaired the policy and procedure committee with oversight of organization, development, revisions and approval of hospital-wide policies. 

Other positions she has held at Chino Valley Medical Center include Intensive Care Unit/Cardiac Cath Lab Director and Magnet Program Director.  For five years, she was Director of Operations/Education Coordinator for Flex Ed in Chino, overseeing administrative daily operations in California and Las Vegas for a health care education organization.  She also serves as Adjunct Nursing Professor for West Coast University in Ontario. 

Ms. Grelling earned her BSN from Azusa Pacific University and her Masters of Nursing and Family Nurse Practitioner from California State University, Long Beach. 

Vanessa Perez, PhD

Behavioral Health Director

Dr. Vanessa Perez is a first Generation bilingual Mexican American who completed her undergraduate Degree in Psychology and Chicano/a Studies at UCLA and went on to complete her Graduate studies in Forensic Psychology at Alliant International University.  Her emphasis has been working with populations who are typically underserved and underprivileged in an attempt to introduce healthier coping skills and core acceptance, which is accredited to her own humble beginnings.

Her experience includes providing individual, group, and family therapy to incarcerated adolescents and adults, as well as supportive and educational treatment to their families; children and families involved with Department of Children and Family Services; elderly in Skilled Nursing Facilities; and currently in providing services in the clinic setting and Rehabilitation Hospital arena. Vanessa has been recognized for her collaborative efforts in partnering with local hospital’s Disaster Planning teams ensuring inclusion of People with disabilities, chronic conditions, and special needs. 

Sue Montoya-Bell

Case Management Director

Sue Montoya-Bell has a Bachelor’s degree in Social Work from Cal Poly Pomona, a Master’s degree in Social Work from USC and post graduate education from Cal State Fullerton and Cal State San Marcos. Sue has been a licensed clinical social worker since 1987 and has post graduate certificates in gerontology, case management and palliative care.

She has worked in the social service field since 1980 when she volunteered at the youth prison (YTS) in Chino and completed an undergraduate internship at CPS. Since that time, Sue has worked approximately 12 years in community mental health and private practice. She also worked 5 years in Home Health services, and nearly 28 years in an acute hospital setting providing social service and case management services. In most of her jobs, she has also provided administrative and clinical oversight to the departments she has worked in. She has been involved with multiple schools of Social Work, providing clinical supervision for interns from Cal Poly Pomona, Azusa Pacific, USC, Loma Linda, Cal State LA, Cal State Long Beach and Cal State San Bernardino.

Sue has a passion for serving the community and was recognized in 2016 Inland Empire Magazine as a “Woman of Distinction” for her extensive outreach and collaborations with various community programs serving the area’s neediest populations. Because of Sue’s skills in addressing needs from the micro to the macro levels, she has been instrumental in enhancing HSA’s community partnerships to improve the overall continuum of care.

J. Michael Finley DO FACOI, FACP, FACR

Chief Medical Officer

J. Michael Finley DO is an independent clinician – educator with expertise in ambulatory medical care and education of medical students, residents and practicing physicians. Dr. Finley received his Bachelor of Science degree in microbiology and public health from Michigan State University and DO degree from Chicago College of Osteopathic Medicine, now Midwestern University. He completed internal medicine residency at Detroit Osteopathic Hospital in Highland Park, Michigan and a rheumatology fellowship at the Loma Linda University Medical Center in Loma Linda, California.

Dr. Finley most recently was Senior Associate Dean for Academic Affairs at the Kansas Health Science Center/Kansas College of Osteopathic Medicine. At KHSC he successfully guided KansasCOM’s Candidate and Preaccrediation applications with the Commission on Osteopathic College Accreditation, hired the faculty team, and established the curriculum. In 2018, Dr. Finley served as the inaugural Senior Vice President for Assessment at the National Board of Osteopathic Medical Examiners (NBOME) in Chicago, Illinois. Prior to moving to NBOME, he retired as Associate Professor of Internal Medicine and Associate Dean, Graduate Medical Education at Western University/COMP, Pomona, California, after 18 years of service. While at WesternU, Dr. Finley worked as Chief Academic Officer and ACGME Designated Institutional Official for OPTI – West, an Educational Consortium sponsor of 18 ACGME residency programs. Under Dr. Finley’s direction more than 40 new residency training programs in California, Nevada, Oregon and Utah were developed. He is past Chair of the COMP Department of Internal Medicine and was founding Internal Medicine residency program director at Arrowhead Regional Medical Center in Colton, California from 2004 – 2012. As a clinician, he specializes in adult ambulatory Internal Medicine.

Dr. Finley has been member of the faculty of UCLA School of Medicine, Loma Linda University School of Medicine and currently is an invited lecturer at the UC Riverside School of Medicine. Dr. Finley served as a member of the United States Food and Drug Administration, Arthritis
Advisory Committee from 2004 – 2006. In 2015, he was recognized as W. Douglas Ward PhD Educator of the Year by the American Osteopathic Foundation.

Dr. Finley is the proud father of two adult sons, RJ and Casey, and is a resident of Orange County, California where he lives with his wife of 32 years, Shelley, and Black Labrador, Carley.

Chino Valley Community Health Center

CVCHC is located near the center of Chino, providing convenient access to the communities of Chino Hills, Corona, Ontario, and Pomona. Services provided here include behavioral health and medical. We also have an onsite pharmacy for the convenience of our patients.

Montclair Community Health Center

MCHC is located near the 10 freeway on the West End of San Bernardino County. Services provided here include behavioral health, medical, radiology, and urgent care. We also have an onsite pharmacy for the convenience of our patients.

The S. Newandee MD Community Health Center

SNMDCHC is located in Rancho Cucamonga on Foothill Blvd. near Archibald Ave. Services provided here include behavioral health and medical.

Palm Desert Community Health Center

Formerly known as Foothills Psychological Services, PDCHC is located on Cook St. south of the 10 and north of the 111, providing convenient service to Palm Desert, Rancho Mirage, Indian Wells and surrounding areas. Services provided at this location are for behavioral health.

Alta Loma Community Health Center

ALCHC is located south of the 210 and north of Foothills Boulevard, west of Hellman Ave. Services provided here include behavioral health and medical.

Rancho Cucamonga Community Health Center

RCCHC is located just south of Foothill Blvd. east of Haven Ave. on Laural St. Services provided at this location are for behavioral health and medical.
